PRESERVE Awake Breast Augmentation: Why Preserving Natural Breast Anatomy May Support a More Durable Result
Preserve breast augmentation elongates and redistributes your own tissues around the implant instead of cutting them, so your natural support system keeps working for you.
Dallas plastic surgeon Dr. John Burns explains ligaments, bottoming out, smaller implants and what “long lasting” really means.

The short answer
Preserve breast augmentation creates the implant space by stretching and redistributing your own tissues with a balloon instead of extensively cutting them. Because more of the breast’s ligaments, fascia and natural support stay intact, they can keep helping hold the implant in position, which may support a more durable, long-lasting result.

Breast augmentation has traditionally focused on the implant: its size, shape, profile, and position. With PRESERVE breast augmentation, the philosophy is different. The operation is designed around the idea that the patient’s native breast anatomy is valuable and should be preserved whenever possible.
At the practice of John Burns, MD, FACS, PRESERVE is especially appealing for patients seeking a natural result, rapid recovery, and in selected cases an awake breast augmentation using local anesthesia or light sedation. But one of the most interesting potential advantages goes beyond the immediate recovery: by minimizing internal tissue cutting and preserving the natural support system of the breast, PRESERVE is designed to allow the patient’s own tissues to continue helping support the implant over time.
That may have important implications for long-term breast stability.

The Breast Already Has a Natural Support System
The breast is not simply skin filled with breast tissue.
It is supported by an intricate network of:
- connective tissue
- fascia
- fibrous septa
- skin attachments
- the inframammary fold
- ligaments surrounding and supporting the breast envelope
Among these are the circummammary ligaments, which help define and support the breast around its perimeter.
These structures contribute to breast shape and help maintain the relationship between the breast, chest wall, and inframammary fold.
Traditional augmentation techniques may require more extensive internal dissection to create a pocket. That can involve cutting through some of the tissues that normally help support the breast.
PRESERVE is designed to approach pocket creation differently.
The Core PRESERVE Concept: Elongate and Redistribute Instead of Cut
Motiva describes PRESERVE as a less-invasive breast tissue preservation technique in which the native tissues are elongated and redistributed around the implant rather than extensively cut. A specialized channel separator creates a pathway, followed by controlled expansion with an inflatable balloon to develop the implant space while minimizing disruption of the native supporting structures. (Motiva)
This is an important conceptual difference.
Instead of creating a pocket primarily by surgically dividing tissue, the technique is intended to create a three-dimensional space by stretching, separating, and recruiting the existing tissues.
The implant is then placed into what can be thought of as a native tissue nest.
That means the implant is not simply sitting in an empty space created by surgical dissection. The surrounding breast tissues remain present and are intended to contribute to implant support.

The Role of the Circummammary Ligaments
The circummammary ligament system is particularly relevant to the concept of long-term implant stability.
These ligamentous structures help define the natural perimeter and support of the breast. When they are preserved, they may continue functioning as part of the breast’s native support system.
PRESERVE is designed to minimize disruption of these structures and instead use controlled tissue expansion to incorporate them into the newly created implant pocket.
Motiva specifically describes the inflatable balloon technique as recruiting native tissues and ligaments around the implant to help provide stability. (Motiva)
This is one of the reasons the technique is so different from simply calling an implant “above the muscle.”
The important issue is not just where the implant is placed.
It is how the space for that implant is created.
How Preserve Breast Augmentation Compares With Traditional Augmentation

Why Cutting Less Tissue May Matter Over Time
Every time internal breast tissue is divided, some portion of the native architecture is altered.
That may be necessary in many traditional breast operations, and conventional augmentation remains an excellent procedure when performed properly. But from a preservation standpoint, there is an intuitive advantage to leaving healthy tissue intact when it does not need to be cut.
PRESERVE is built around that principle.
By minimizing internal tissue division, the procedure aims to retain more of the breast’s:
- connective tissue support
- ligamentous attachments
- soft-tissue architecture
- sensation
- vascularity
- natural compliance
The theoretical long-term advantage is straightforward:
The more of the native breast support system that remains intact, the more of that support system remains available to help hold the implant in position.
A Native “Hammock” Around the Implant
One way to think about PRESERVE is that the patient’s own tissues form a biologic hammock or nest around the implant.
Rather than removing or dividing the very tissues that could provide support, the procedure attempts to preserve and redistribute them around the device.
Motiva reports that in an early clinical study of the PRESERVE technique, there was a 0% rate of inferior implant malposition at three years. (Motiva)
That is encouraging evidence for the concept of stability, although three-year data should not be confused with proof of lifetime superiority. Breast augmentation outcomes are influenced by many factors, including implant size, skin quality, pregnancy, weight change, aging, tissue elasticity, and patient anatomy.
Still, the early stability data are consistent with the core philosophy of PRESERVE: preserve the natural support structures rather than cutting them whenever possible.
Why This Could Matter for Bottoming Out
One of the long-term concerns with breast implants is inferior malposition, commonly called bottoming out.
Over time, an implant may descend below its intended position if the lower-pole tissues stretch or if support is insufficient.
Factors that can contribute include:
- very large implants
- weak native tissue
- aggressive pocket dissection
- excessive lowering of the inframammary fold
- pregnancy
- major weight fluctuations
- aging
- gravity
A preservation-based technique may theoretically help by minimizing unnecessary disruption of the tissues responsible for lower-pole and peripheral support.
That does not mean PRESERVE makes bottoming out impossible.
No technique can completely eliminate the effects of aging, gravity, or biology.
But preserving the tissues that help support the breast is a fundamentally different approach from cutting those structures and then relying on scar tissue alone for stability.

Why Smaller Implants Pair So Well With PRESERVE
The durability of any breast augmentation is also influenced by implant weight.
A larger implant places greater mechanical load on the breast envelope over time.
PRESERVE works particularly well with the current trend toward smaller, lighter, more natural breast implants.
Motiva’s expansion into smaller implant sizes is especially well suited to this philosophy. A smaller implant can provide beautiful shape and projection while placing less weight on the breast’s supporting tissues. (Read more about Motiva’s new small breast implant sizes.)
The combination of:
smaller implant + preserved tissue + precise pocket + native ligament support
is a compelling strategy for patients interested in maintaining a natural result over the long term.

PRESERVE and the Subfascial Plane
PRESERVE is generally performed in an above-muscle tissue-preserving plane, avoiding disruption of the pectoralis muscle.
This is attractive for several reasons.
Because the muscle is not elevated:
- there is no animation deformity
- chest muscle function is preserved
- athletic patients can maintain more normal muscle mechanics
- recovery can be faster
- the implant moves more naturally with the breast rather than with muscle contraction
In selected patients, the subfascial tissue layer can also contribute an additional layer of implant coverage.
Again, the principle is preservation rather than destruction.

Why PRESERVE Is Well Suited to Awake Breast Augmentation
PRESERVE is also uniquely suited to the concept of awake breast augmentation.
Because the technique is designed to minimize surgical trauma and avoid extensive muscle dissection, selected patients can undergo the procedure using local tumescent anesthesia with or without light sedation.
For many patients, this means:
- no general anesthesia
- minimal bleeding
- less tissue trauma
- less postoperative pain
- rapid range of motion
- faster return to normal activity
- early return to exercise
But awake breast augmentation is not simply about avoiding anesthesia.
The more important concept is that a less disruptive operation may also preserve more of the anatomy that contributes to long-term breast stability.

Preservation May Also Help Maintain Breast Sensation
The same tissue-preserving philosophy may have benefits beyond implant position.
Motiva describes PRESERVE as a technique intended to preserve native breast tissue and sensation over time. (Motiva)
By minimizing extensive internal cutting and avoiding unnecessary disruption of breast tissue, the technique is designed to reduce trauma to the structures that contribute to normal breast sensation and function.
This can be particularly meaningful for younger patients who may want to preserve:
- nipple sensation
- breast sensation
- breastfeeding potential
- normal tissue movement
- future surgical options
For these patients, the goal is not merely to enlarge the breast.
It is to enhance the breast while preserving as much of its natural anatomy as possible.

Does PRESERVE Guarantee the Longest-Lasting Breast Augmentation?
It is important to be scientifically precise.
PRESERVE has a compelling anatomical rationale for long-term stability, and early clinical data showing no inferior malposition at three years are encouraging. (Motiva)
However, there are not yet decades of comparative data proving that PRESERVE produces the longest-lasting breast augmentation of every available technique.
Breasts continue to age after surgery.
Skin stretches.
Pregnancy changes tissue.
Weight fluctuates.
Gravity continues to act.
Implants are also not lifetime devices.
So the better way to describe PRESERVE is this:
It is designed to maximize long-term stability by preserving the native breast support system rather than disrupting it unnecessarily.
That is a strong anatomical principle and one that makes intuitive sense for a procedure intended to last many years.

See the before-and-after gallery.
Preservation Versus Reconstruction
Traditional augmentation often involves creating a pocket and then relying on healing and scar formation to stabilize the implant.
PRESERVE takes a different philosophical approach:
preserve first, reconstruct less.
If the breast’s native ligaments, fascia, and internal architecture are healthy, why remove or divide them unnecessarily?
The goal is to use the patient’s own anatomy as part of the solution.
This is increasingly consistent with modern surgical thinking across many specialties: preserve normal anatomy whenever possible and minimize unnecessary tissue trauma.
Who Is the Ideal PRESERVE Patient?
PRESERVE can be particularly attractive for patients who:
- want a natural breast augmentation
- prefer a smaller or moderate implant
- are athletic or highly active
- want to avoid animation deformity
- want a rapid recovery
- want to avoid general anesthesia when appropriate
- have relatively healthy breast tissues
- have minimal to moderate ptosis
- value preservation of sensation and native anatomy
Patients with severe ptosis, major implant malposition, extensive capsular contracture, or complicated revision anatomy may still require more traditional surgical techniques.
The procedure should always be matched to the anatomy.
If sagging is the main concern, consider a breast lift.
Preserve, a lift or a revision? Let Dr. Burns match it to your anatomy.
Risks and Long-Term Care After Preserve Breast Augmentation
Implant risks still apply. The FDA lists capsular contracture, rupture, infection, seroma, hematoma, malposition and rippling, plus the rare BIA-ALCL, and states implants are not lifetime devices.
ASPS notes implants are often replaced 10 to 20 years after surgery. See can silicone implants last a lifetime?

The Future of Breast Augmentation: Preserve More, Disrupt Less
Breast augmentation is evolving.
The next generation is not simply about a new implant.
It is about a different way of thinking.
Instead of asking:
How much tissue do we need to release to make room for an implant?
PRESERVE asks:
How much native anatomy can we keep intact while still creating a beautiful augmentation?
That distinction may prove increasingly important.
By preserving circummammary ligaments, native breast tissue, fascia, and the natural support architecture of the breast, PRESERVE creates an augmentation that works with the patient’s anatomy rather than replacing it.
At the practice of John Burns, MD, FACS in Dallas, PRESERVE Awake Breast Augmentation represents a modern approach to breast surgery: smaller implants, minimal disruption, no muscle animation, rapid recovery, and an emphasis on maintaining the natural support system of the breast for the best possible long-term stability.
The concept is simple:
The tissue you do not cut is tissue you still have working for you.

Preserve breast augmentation FAQ
What is Preserve breast augmentation?
A less-invasive Motiva technique in which a channel separator and inflatable balloon create the implant space, so native tissues are elongated and redistributed around the implant rather than extensively cut.
How long does Preserve breast augmentation last?
It is designed to maximize long-term stability, and early three-year data are encouraging. Decades of comparative data do not yet exist, and implants are not lifetime devices.
Does Preserve prevent bottoming out?
It does not make bottoming out impossible. Motiva reports a 0% rate of inferior implant malposition at three years in an early study, but aging, gravity, implant size and tissue quality still matter.
Can Preserve be done awake?
Yes, for selected patients, using local tumescent anesthesia with or without light sedation instead of general anesthesia.
Is Preserve over or under the muscle?
It is generally performed in an above-muscle, tissue-preserving plane, so the pectoralis is not elevated and there is no animation deformity.
How long is recovery after Preserve?
Dr. Burns’ Preserve page describes light activity in 1 to 2 days, exercise in about a week and full activity within about 10 days. These are estimates.
Will I keep breast and nipple sensation?
Preserve is intended to preserve native tissue and sensation by minimizing internal cutting, but no technique can guarantee sensation.
Who is not a good candidate for Preserve?
Patients with severe ptosis, major implant malposition, extensive capsular contracture or complicated revision anatomy may still require more traditional techniques.
How much does Preserve cost in Dallas?
Fees depend on your implant, anesthesia and any combined procedures. Dr. Burns provides a personal quote at consultation, and financing is available.
Will my implants need to be replaced?
Possibly. The FDA states breast implants are not lifetime devices, so regular follow-up and recommended imaging matter.
The takeaway
Preserve changes how the implant space is created: tissue is stretched rather than cut, so natural support keeps working. Early three-year data are encouraging, but no technique stops aging, and implants are not lifetime devices. With a smaller implant and healthy tissue, it is a compelling path to a durable result.
