The short answer

A breast lift with implants (augmentation mastopexy) raises and reshapes sagging breasts while an implant restores fullness. In Dr. Burns' Lift and Fill approach, the lift is perfected first, a temporary sizer tests implant volume, and the permanent implant is chosen only after verification.

Wondering if you need a lift, implants or both? Send photos through Dr. Burns' free online consultation for a personal opinion.

Combining a breast lift with implants is one of the most powerful procedures in aesthetic breast surgery. It can completely transform breasts that have lost volume, shape, and youthful position following pregnancy, breastfeeding, weight loss, or aging.

However, combining these two procedures—known as an augmentation mastopexy—is also one of the most technically challenging operations in plastic surgery.

Why? Because a breast lift and breast augmentation accomplish two very different things. A breast lift removes excess skin, elevates the nipple, and reshapes the breast, while an implant adds volume, fullness, projection, and roundness.

Achieving beautiful results requires these two procedures to work in perfect harmony.

At my Dallas plastic surgery practice, I use a surgical philosophy I call “Lift and Fill.” This systematic approach places the breast lift first, prioritizing precise breast measurements, nipple position, symmetry, and shape before determining the final implant size.

The concept is simple: Perfect the lift first. Then select the implant that best complements the newly shaped breast.

This approach helps us achieve natural, balanced, and predictable results without compromising breast shape to accommodate a predetermined implant size.

The challenge of combining a breast lift with implants

Breast augmentation and breast lift surgery are individually well-established procedures. Combining them creates additional complexity because the procedures work in opposite directions. (A breast lift alone does not add much volume.)

A breast lift tightens the breast skin envelope and repositions breast tissue upward. An implant increases volume and places additional tension on that same skin envelope.

Historically, some approaches have placed the implant first, then tailored the breast lift around the implant.

While implant-first techniques remain appropriate in selected patients, they may create a challenge: The final lift can become influenced by the implant's size rather than being designed exclusively around ideal breast anatomy.

A larger implant can create temporary fullness, but it cannot independently correct significant sagging, poor nipple position, or excess skin.

The Lift and Fill approach changes the emphasis.

Instead of making the breast fit the implant, we select an implant that fits the newly lifted breast.

Step 1: The lift comes first

The first and most important principle of Lift and Fill is that the breast lift takes priority.

Before selecting the final implant, the goal is to create a beautifully shaped breast with carefully controlled anatomical measurements.

Every detail matters.

Precise nipple position

The nipple-areola complex should be positioned at an aesthetically pleasing height, centered on the breast mound and aligned with the newly established breast contour.

Too high, too low, too medial, or too lateral can negatively affect the final appearance.

A well-designed breast lift restores nipple position in proportion to the patient's individual anatomy.

Breast symmetry

Most women have some degree of natural breast asymmetry.

One breast may be larger, wider, lower, or differently shaped than the other.

During the lift, these differences are addressed by adjusting the skin envelope and reshaping the underlying breast tissue.

The goal is to create the most symmetrical breast foundation possible before adding implant volume.

Ideal breast measurements

Several important measurements guide the lift:

  • Sternal notch-to-nipple distance: Helps determine proper nipple elevation and breast position.
  • Nipple-to-inframammary fold distance: Establishes an appropriate lower-pole breast dimension.
  • Breast base width: Determines the ideal breast footprint and helps guide implant selection.
  • Breast projection: Defines how far the breast extends from the chest wall.
  • Areolar size and position: Ensures the nipple-areola complex is proportionate to the breast.
  • Breast shape and skin envelope: Establishes a balanced, youthful contour.

The lift is carefully planned to optimize these measurements while maintaining healthy blood supply and appropriate tissue tension.

The objective is to create a beautiful breast shape independent of the implant.

Step 2: The temporary implant sizer—test before you commit

Once the breast has been appropriately lifted and reshaped, the next step is determining the ideal implant volume.

This is where the Lift and Fill algorithm becomes especially valuable.

Instead of immediately opening and placing a permanent breast implant, I use a temporary breast implant sizer.

A sizer is a sterile, temporary device designed to simulate the dimensions and volume of a permanent implant.

It allows me to evaluate the actual appearance of the lifted breast with additional volume before committing to the final implant.

This is an important distinction.

We are no longer estimating how the breast might look with an implant.

We can directly assess how the newly lifted breast responds to a particular implant size.

With the temporary sizer in place, I evaluate:

  • Upper-pole fullness and cleavage
  • Overall breast projection
  • Breast width and contour
  • Lower-pole shape
  • Symmetry between the breasts
  • Nipple position relative to the augmented breast mound
  • Skin tension and tissue support
  • Balance between implant volume and the patient's natural breast tissue

The patient can also be assessed in a more upright operating-table position to help evaluate breast shape and symmetry.

If the breast needs more fullness, we can test a larger volume. If an implant appears too large, too tight, or disproportionate, we can evaluate a smaller size.

This ability to test and refine the implant selection is one of the greatest advantages of the Lift and Fill approach.

Step 3: The permanent implant is selected only after verification

One of the defining principles of my Lift and Fill technique is that the permanent implant is not opened until the result has been evaluated with the temporary sizer.

This eliminates much of the guesswork associated with implant selection.

Although preoperative measurements, patient preferences, and 3D imaging provide an excellent starting point, nothing replaces direct intraoperative assessment.

The sizer allows us to verify that the selected implant volume complements the breast lift rather than compromises it.

After confirming the appropriate size and profile, the temporary sizer is removed, the implant pocket is prepared, and the final permanent implant is inserted using meticulous sterile technique.

The goal is to choose an implant that creates beautiful fullness while preserving the shape and support achieved by the lift.

The implant should enhance the lift—not dictate the lift.

Step 4: Creating the perfect balance between lift and volume

A beautiful augmentation mastopexy requires more than lifting the nipple and making the breasts larger.

The ideal result combines several elements:

  1. Youthful position: The breasts sit higher on the chest with the nipples appropriately centered.
  2. Beautiful shape: The breast mound is rounded, smooth, and proportionate.
  3. Upper-pole fullness: The implant restores fullness in the upper breast, which frequently disappears after pregnancy or weight loss.
  4. Symmetry: Both breasts are balanced in position, size, and contour.
  5. Natural proportions: The implant size complements the patient's chest width, breast tissue, and overall figure.
  6. Long-term support: The breast tissue and implant are supported to help maintain their position over time.

When these components are carefully coordinated, the result can be transformative.

The breasts look lifted, fuller, rounder, and more youthful without appearing excessively large or artificially shaped.

Why bigger implants are not always better

One of the most common misconceptions about breast lift surgery is that a larger implant will automatically produce a better result.

In reality, excessively large implants can work against the goals of a breast lift.

Larger implants add more weight and tension to the breast skin and supporting tissues. In patients with already stretched or weakened breast tissue, excessive implant volume may contribute to recurrent sagging, bottoming out, or implant displacement over time.

With Lift and Fill, the priority is to determine how much volume the newly lifted breast can appropriately accommodate.

Sometimes a moderate-sized implant creates a much more attractive result than a larger one.

Other patients may benefit from greater projection or fullness.

The advantage is that implant size is selected based on the actual breast shape achieved during surgery—not solely on a predetermined number of cubic centimeters.

The right implant is not necessarily the biggest implant. It is the implant that creates the best result.

Adding an internal mesh bra for long-term support

For selected patients, I combine Lift and Fill with an internal mesh bra to provide additional support to the lower breast and implant.

An internal bra uses a specialized surgical mesh, such as GalaFLEX, to reinforce the breast's supporting structures.

This can be especially beneficial in patients with thin skin, significant breast sagging, weakened breast tissue following pregnancy, or previous implant-related problems.

The mesh acts as a reinforcing scaffold while the tissues heal and remodel.

By combining precise breast reshaping, carefully selected implant volume, and appropriate internal support, we aim to create an attractive result that maintains its shape over time.

However, no technique can completely prevent future changes caused by aging, gravity, weight fluctuations, or pregnancy.

Lift and Fill with Motiva breast implants

Implant technology has also advanced significantly in recent years.

In my practice, I frequently use Motiva breast implants because of their range of profiles, advanced implant design, and ability to create soft, natural-looking breast contours. (See Motiva vs. traditional silicone implants and the FDA breast implant information.)

The Lift and Fill technique pairs well with modern implant options because the implant can be chosen to complement the already established breast shape.

Whether the patient desires a subtle enhancement or a fuller, more rounded appearance, selecting the proper implant width, projection, and volume is essential.

For many patients, a moderate-profile implant produces a beautiful balance between natural proportions and attractive upper-pole fullness.

Who is a good candidate for Lift and Fill?

The Lift and Fill approach may be particularly beneficial for women who have experienced significant changes in their breasts.

Common candidates include:

  • Mothers after pregnancy and breastfeeding: Women with deflated upper breasts, stretched skin, and nipple descent.
  • Patients after major weight loss: Women who have lost breast volume and developed significant skin laxity.
  • Women with naturally sagging breasts: Patients who want both nipple elevation and increased fullness.
  • Patients with breast asymmetry: Women who need customized adjustments to breast shape and implant volume.
  • Women undergoing breast implant revision: Patients who need implant replacement combined with correction of sagging, asymmetry, or stretched breast skin.

In carefully selected patients, a breast lift and augmentation can be performed during the same surgery. In more complex cases involving significant skin laxity, large desired implants, or compromised tissue, a staged approach may provide a safer or more predictable option.

Related options: a mommy makeover, a breast lift with fat transfer, or breast implant revision.

What makes Lift and Fill different from traditional augmentation mastopexy?

The central difference is the order of priorities.

Traditional implant-first approach

In many established augmentation mastopexy techniques, the implant or implant sizer is placed early in the operation, and the breast lift is then adjusted around the augmented breast.

This technique can work well and remains a valid surgical option.

The Lift and Fill approach

In my Lift and Fill algorithm, the lift is the primary operation.

We establish breast position, nipple alignment, lower-pole dimensions, skin-envelope shape, and symmetry as the foundation of the final result.

A temporary implant sizer is then used to determine the volume that best complements this newly shaped breast.

Only after this evaluation is complete do we select and open the permanent implant.

This sequencing emphasizes anatomy, precision, and individualized implant selection.

The concept reflects an important trend in modern aesthetic breast surgery: moving away from simply adding volume toward creating a carefully measured, harmoniously shaped breast.

Although temporary sizers and measurement-based algorithms are established surgical tools, the specific Lift and Fill sequence described here represents my approach to optimizing the combined procedure.

The importance of experience in breast lift with implant surgery

Augmentation mastopexy requires advanced judgment and technical precision.

The surgeon must simultaneously manage skin removal, nipple elevation, breast tissue reshaping, implant pocket creation, implant size, and soft-tissue support.

Each of these decisions influences the others.

With more than 25 years of experience in aesthetic breast surgery, I have found that the most reliable approach is one that minimizes compromise and allows each component of the operation to be carefully evaluated.

My Lift and Fill philosophy places emphasis on creating the correct breast anatomy first, then using the implant to refine volume and projection.

It is a thoughtful, systematic approach designed to make an inherently complex procedure more controlled and predictable.

As with all augmentation mastopexy procedures, outcomes depend on tissue quality, healing, surgical technique, and individual anatomy. Risks include scars, wound-healing problems, changes in nipple sensation, asymmetry, implant complications, recurrent ptosis, and possible revision surgery.

The future of beautiful breast lift with implant surgery

Modern breast surgery is evolving beyond the traditional concept of simply making breasts bigger or lifting them higher.

The goal is to create a beautiful breast with the right shape, position, volume, and proportions.

That is the philosophy behind Lift and Fill.

By prioritizing the breast lift, perfecting anatomical measurements, using temporary implant sizers to evaluate volume, and selecting the final implant only after careful verification, we can take a more deliberate approach to one of plastic surgery's most challenging procedures.

For me, Lift and Fill is about removing unnecessary guesswork and replacing it with precision.

The result is a surgical strategy designed to create breasts that are beautifully lifted, naturally full, symmetrical, and proportionate to each individual patient.

Lift first. Size second. Fill with precision.

That is the foundation of the Lift and Fill approach to modern augmentation mastopexy.

Frequently asked questions

What is a breast lift with implants?

Also called augmentation mastopexy, it combines a breast lift, which removes excess skin, raises the nipple and reshapes the breast, with an implant that adds volume and upper-pole fullness.

How much is a breast lift with implants in Dallas?

Dr. Burns' breast lift page lists approximately $12,000, depending on the implant selected. Prices vary and are not exact; your personal quote is provided at consultation.

How is a breast lift done with implants using Lift and Fill?

The lift is performed first. A temporary sizer then tests implant volume, and the permanent implant is opened only after the result is verified.

Do you have to get implants with a breast lift?

No. A lift alone raises and reshapes the breast but adds little fullness. Implants, or fat transfer for some patients, are added when you also want more volume.

Can you get a breast lift and implants in the same surgery?

Often, yes. With significant skin laxity, large desired implants or compromised tissue, a staged approach may be safer or more predictable.

How painful is a breast lift with implants?

Soreness, swelling and mild bruising are expected for several days. Many patients return to desk work within 2 days to 1 week.

What size implant should I get with a breast lift?

Not necessarily the biggest. Overly large implants may contribute to recurrent sagging or bottoming out, so size is chosen to fit the newly lifted breast.

What are the risks of a breast lift with implants?

Scars, wound-healing problems, changes in nipple sensation, asymmetry, implant complications, recurrent ptosis and possible revision surgery.

The takeaway

A breast lift with implants restores position, shape and fullness at once, but the two procedures pull in opposite directions. Lift and Fill resolves that: perfect the lift, test volume with a sizer, and open the permanent implant only after verification. A consultation shows whether a lift, implants, both or a staged plan fits you.
